Please read these Terms of Service carefully before using momdoc.com or any MomDoc practice website. By accessing or using these sites, you agree to be bound by these Terms.

About MomDoc

MomDoc is a women's health practice based in Arizona, serving patients across multiple offices and sub-practices (MomDoc, Women For Women, MomDoc Midwives, Mi Doctora, and Women's Health Research). References to "we," "us," or "our" in these Terms mean MomDoc and its affiliated practices.

Not Medical Advice

The content on this website is for general informational and educational purposes only. It is not medical advice, does not constitute a diagnosis or treatment plan, and is not a substitute for the judgment of a qualified healthcare provider.

Using this site, reading our articles, or filling out a form on the site does not create a patient-provider relationship with MomDoc. A relationship is established only when you become a patient of record through our intake process.

Governing Law and Venue

These Terms are governed by the laws of the State of Arizona, without regard to conflict-of-law principles. Any dispute arising out of or related to these Terms or your use of this site will be resolved in the state or federal courts located in Maricopa County, Arizona.

Changes to These Terms

We may update these Terms from time to time. When we do, we will revise the "Effective" date at the top of this page. Your continued use of the site after the effective date of an updated version constitutes your acceptance of the changes. For material changes, we will make reasonable efforts to notify patients and site users through the site, by email, or at their next visit.
