Billing Transparency
Annual Exam vs. Problem Visit
Insurance covers preventive care (your annual Well Woman exam) at 100%. If you discuss a specific medical problem during the same visit, insurance rules require a diagnostic code, which may trigger a copay or deductible.
How Pregnancy Billing Works
Maternity care uses a "Global Fee" model. Your routine prenatal care, delivery, and postpartum checkup are bundled into one package. We confirm your benefits early in your pregnancy.
Lab Tests Are Separate
Pap smears, blood work, and biopsies are processed by external labs (Sonora Quest or LabCorp), not MomDoc. You will receive a separate bill from the lab. Tell your provider if your insurance requires a specific lab.
Good Faith Estimate
Uninsured or self-pay? You have the right to a Good Faith Estimate of expected charges before scheduling non-emergency services. Ask our team when you book.
Financial Assistance & Government Programs
AHCCCS (Arizona Medicaid)
We accept all major AHCCCS plans including BCBS Health Choice, Mercy Care, Arizona Complete Health Complete Care Plan, UHC Community Plan, Molina Complete Care, Banner University, and Ambetter. If you are uninsured and think you may qualify, our on-site Health-e-Arizona Assistor can help you apply at no cost.
Medicare
We accept Medicare and most Medicare Advantage plans. Please call (480) 821-3601 to verify your specific plan is accepted.
Federal Emergency Services (FES)
MomDoc accepts FES for maternity care. FES primarily covers your hospital delivery, but we believe every baby deserves a healthy start — we provide comprehensive prenatal care for FES patients. At MomDoc, all women are seen.
Sliding Fee Scale
Uninsured or underinsured? Our sliding scale is based on Federal Poverty Guidelines (family size + income). Proof of income required, applied at time of service. Call (480) 821-3601 to ask about it.
CommunityCares: Social Needs Support
Health does not start and end at the clinic door. If you need help with food, housing, transportation, childcare, utility bills, or other needs that affect your well-being, MomDoc participates in CommunityCares, Arizona's statewide social care network.
Our team can send a secure referral to a local community organization on your behalf and follow up to make sure you received help. There is no cost to you, and no paperwork for you to navigate.
